Advance Directives Workshop: The Five Wishes

Two Sessions In Person at Berkshire Community College
Tuesday, May 23 at 1:00 - 4:00 p.m.
and
Tuesday, May 30 at 1:00 - 4:00 p.m.
Using a supportive, facilitated, group format this workshop will enable participants to understand and complete The Five Wishes living will, an advanced directive document. The workshop will be offered in two three-hour sessions with a notary public present for certification of the documents of each participant at the completion of the second session.
Five Wishes is the first living will (also called an Advanced Directive) that talks about your personal, emotional and spiritual needs, as well as medical wishes. It lets you chose the person you want to make your health decisions if you are not able to make them yourself. It was written with the help the nation’s leading experts in end-of-life care.
Gail has been facilitating groups throughout her career as a Registered Nurse, Certified Nurse-Midwife, Advanced Practice Registered Nurse, and presently as a Community Health Educator for the “Power of Healthcare Advocacy, during the Retirement years.” She is an independent Healthcare Advocate, Phillips Healthcare Advocate, whose mission is to promote and empower “person-centered care” for the best possible outcomes within our healthcare system. With her devotion to this concept in health care, she is also passionate about and promotes the concept of home care, whenever possible, to ensure advocacy-focused kind care.
